01 The premise

Escalation happens on a timer rather than when a manager happens to look. None of it is sophisticated and all of it is the difference between a CRM that pays for itself and one that is resented.

Round-robin is fair and ignores workload. Weighting by current open deals is usually better in practice.
Leave, shifts and working hours. Assigning to somebody who is away is worse than not assigning.
When no rule matches, or the assignee does not respond within a window. Reassignment on silence protects the lead.
Everyone sees who owns what and why. Opaque assignment rules produce resentment and quiet non-compliance.
